Resistant Hypertension Management

Assessment and Diagnosis

  • The American Heart Association recommends that patients with resistant hypertension should be evaluated for secondary causes, including obstructive sleep apnea (OSA), which is a common cause of resistant hypertension 1
  • Patients with uncontrolled hypertension should be assessed for medication adherence, as poor adherence is a primary barrier to blood pressure control 1
  • The International Society of Hypertension recommends that patients with resistant hypertension should undergo a thorough diagnostic evaluation, including a sleep study to rule out OSA 2

Treatment and Management

  • The American College of Cardiology recommends that patients with resistant hypertension should be treated with a combination of lifestyle modifications and pharmacological therapy, including the use of ACE inhibitors, beta-blockers, and diuretics 1
  • The European Society of Hypertension recommends that patients with resistant hypertension should be considered for referral to a hypertension specialist if blood pressure remains uncontrolled despite the use of multiple medications 3
  • The American Academy of Sleep Medicine recommends that patients with OSA should be treated with continuous positive airway pressure (CPAP) therapy, which can help to reduce blood pressure and improve overall health outcomes 1

Medication Management

  • The American Heart Association recommends that patients with resistant hypertension should be treated with a simplified medication regimen, including the use of single-pill combination therapy to improve adherence 1
  • The International Society of Hypertension recommends that patients with resistant hypertension should be treated with a combination of medications, including ACE inhibitors, beta-blockers, and diuretics, to achieve optimal blood pressure control 2
  • The European Society of Hypertension recommends that patients with resistant hypertension should be monitored for hyperkalemia when combining ACE inhibitors and spironolactone, and that potassium levels should be checked within 1 week of initiating therapy 3

Lifestyle Modifications

  • The American College of Cardiology recommends that patients with resistant hypertension should be counseled on lifestyle modifications, including weight loss, dietary changes, and increased physical activity, to help improve blood pressure control 1
  • The European Society of Hypertension recommends that patients with resistant hypertension should be encouraged to adopt a Mediterranean-style diet, which can help to reduce blood pressure and improve overall health outcomes 2
  • The American Academy of Sleep Medicine recommends that patients with OSA should be counseled on sleep hygiene practices, including avoiding caffeine and electronics before bedtime, to help improve sleep quality and reduce blood pressure 1
