Management of Low Serum Iron with Normal Ferritin and TIBC

Understanding the Iron Profile Pattern

  • Low serum iron, normal ferritin, normal TIBC, and low transferrin saturation is not consistent with classic iron deficiency anemia, which typically shows low ferritin, elevated TIBC, and low transferrin saturation, according to the American Journal of Kidney Diseases 1, 2.
  • Normal ferritin levels suggest adequate iron stores, even with low serum iron levels, as stated by the American Journal of Kidney Diseases 1, 3.
  • Low transferrin saturation with normal TIBC suggests a functional iron deficiency or iron sequestration rather than absolute iron deficiency, as reported by the American Journal of Kidney Diseases 2.

Diagnostic Considerations

  • This pattern may indicate anemia of chronic disease/inflammation, as suggested by the Journal of the National Comprehensive Cancer Network 4.
  • Chronic inflammation or infection should be evaluated as underlying conditions, according to the Journal of the National Comprehensive Cancer Network 4 and the American Journal of Kidney Diseases 5, 2.
  • Do not increase oral iron dosage as this is unlikely to improve iron status when ferritin is normal, as recommended by the American Journal of Kidney Diseases 5, 3.
  • Ensure administration of oral iron without food for better absorption, as stated by the American Journal of Kidney Diseases 5.

Monitoring Recommendations

  • Monitor iron parameters every 3 months while on therapy, as suggested by the American Journal of Kidney Diseases 3.
  • Assess response based on hemoglobin improvement rather than just iron parameters, as recommended by the American Journal of Kidney Diseases 2.
  • Failing to investigate underlying causes of abnormal iron metabolism should be avoided, according to the Journal of the National Comprehensive Cancer Network 4.
