Evaluation and Management of Severe Fatigue in Males

Initial Assessment and History

  • The American College of Physicians recommends screening fatigue severity using a 0-10 numeric scale, and if the score is ≥4, proceed with a focused evaluation targeting treatable medical conditions, psychiatric disorders, and contributing factors rather than extensive laboratory testing 1, 2
  • The National Comprehensive Cancer Network suggests using a numeric rating scale (0-10) to assess fatigue severity, asking patients to rate their fatigue over the past 7 days 3, 1
  • Scores of 0-3 require only routine rescreening at regular intervals, while scores ≥4 warrant immediate focused evaluation, and scores ≥7 typically indicate marked functional impairment 3, 1, 2

Focused History and Physical Examination

  • A complete medication review is essential, as side effects are commonly overlooked, and contributing factors such as alcohol and substance use, and anxiety symptoms should be assessed 3, 2
  • The physical examination should include a lymph node assessment and hepatosplenomegaly evaluation 1

Laboratory Workup and Imaging Studies

  • Initial screening tests should include a complete blood count with differential, comprehensive metabolic panel, thyroid-stimulating hormone, and erythrocyte sedimentation rate, and C-reactive protein for inflammation 1
  • Imaging studies such as chest X-ray, CT chest/abdomen/pelvis with contrast, and PET/CT should only be pursued if red flags are present 1

Treatment Approach

  • A structured physical activity program with stretching and aerobic exercise 2-3 times weekly for 30-60 minutes is recommended for all patients, regardless of cause 4, 5, 6
  • Psychostimulants and dietary supplements are not recommended due to lack of efficacy 4, 5, 6
