Acute Appendicitis Management and Treatment

Diagnosis and Evaluation

  • The American College of Surgeons recommends the use of the Adult Appendicitis Score (AAS) to increase diagnostic accuracy, with patients having a high score (AAS ≥ 16) potentially not requiring preoperative imaging 4
  • The World Journal of Emergency Surgery suggests the use of the Score de Alvarado, which includes symptoms such as migratory pain, anorexia, nausea/vomiting, tenderness in the right lower quadrant, rebound tenderness, fever, and leukocytosis, to aid in diagnosis 3

Surgical Management

  • The World Journal of Emergency Surgery recommends laparoscopic appendectomy as the preferred treatment for acute appendicitis, due to its significant advantages over open appendectomy, including less pain, lower incidence of surgical site infection, shorter hospital stay, and faster return to work 1
  • The American College of Surgeons recommends that surgery be performed within 24 hours of admission, with a single preoperative dose of broad-spectrum antibiotics administered 0-60 minutes before surgical incision 2

Non-Operative Management

  • The World Journal of Emergency Surgery suggests that antibiotic treatment may be considered as a safe alternative to surgery in selected patients with non-complicated acute appendicitis and no appendicolith 1
  • The World Journal of Emergency Surgery recommends initial intravenous antibiotics with subsequent conversion to oral antibiotics based on the patient's clinical condition 3

Complicated Appendicitis

  • The American College of Surgeons recommends early appendectomy within 8 hours for complicated appendicitis 3
  • The World Journal of Emergency Surgery suggests that laparoscopic management is associated with fewer readmissions and fewer additional interventions than conservative treatment 2

Special Considerations

  • The American College of Surgeons recommends simple ligation of the appendicular stump over inversion of the stump in both open and laparoscopic appendectomy 2
  • The World Journal of Emergency Surgery recommends routine histopathology after appendectomy to identify unexpected findings 2

Antibiotic Recommendations

  • The World Journal of Emergency Surgery recommends Metronidazole 500 mg every 6 hours + Vancomycin 25-30 mg/kg loading dose, then 15-20 mg/kg every 8 hours for complicated intra-abdominal infections 5
