HBV DNA Monitoring Guidelines After Hepatitis B Treatment
Monitoring During Active Treatment
- The American Association for the Study of Liver Diseases recommends measuring HBV DNA every 1-6 months during the first year of nucleos(t)ide analogue treatment, and every 3-6 months thereafter, even after achieving undetectable levels 1, 2, 3
- For patients on peginterferon alfa therapy, measure HBV DNA at 1-3 month intervals throughout the 48-week treatment course, and check liver function tests monthly 4, 1, 2, 3
- The European Association for the Study of the Liver suggests monitoring HBeAg/anti-HBe status every 3-6 months during nucleos(t)ide analogue treatment 1, 2
Monitoring After Treatment Cessation
- The American Association for the Study of Liver Diseases recommends measuring HBV DNA every 1-3 months during the first year after stopping treatment, and every 3-6 months thereafter 4, 1
- Continue liver function testing every 3-6 months after treatment cessation 4, 1
- Maintain HCC surveillance with ultrasound ± AFP every 6 months in high-risk patients, as recommended by the American College of Gastroenterology 4, 6
Special Monitoring Situations
- After HBeAg seroconversion, retest HBV DNA 2-3 months later to confirm sustained response, and continue monitoring every 3-6 months 4
- In patients with cirrhosis, never discontinue treatment in decompensated cirrhosis, and consider lifelong treatment for compensated cirrhosis patients 1, 7
