Risperidone Dosing Guidelines
Adult Dosing
- The British Journal of Psychiatry recommends a target dose of 4-6 mg/day for adults with schizophrenia, providing the best balance of efficacy and tolerability 1
- For first-episode psychosis in adults, the target dose is 2 mg/day, with a maximum recommended dose of 4-6 mg/day, equivalent to 4-6 mg haloperidol 1, 2, 3
- In adults with delirium, the starting dose is 0.5 mg PO or SC stat, with PRN dosing of 0.5-1 mg q1h PRN, and lower doses (0.25-0.5 mg) are recommended for older or frail patients 4
- For PTSD-related nightmares in adults, the dose range is 0.5-3 mg/day, with 1-3 mg/day showing efficacy in case series, and an average effective dose of 1 mg/day in burn patients 5, 6, 7, 8
Pediatric Dosing
- In children with disruptive behaviors and intellectual disability, the mean effective dose is 1.16 mg/day for conduct problems, with a dose range studied of up to 2.9 mg/day, and most studies show efficacy starting within 2 weeks 9
Special Populations
- In elderly patients, lower starting doses (0.25-0.5 mg) are recommended, with gradual titration, and dose reduction in hepatic impairment 4
Key Clinical Considerations
- The British Journal of Psychiatry recommends slower titration for first-episode patients, elderly patients, and young patients to minimize side effects and improve adherence 1
- The risk of extrapyramidal symptoms increases significantly above 6 mg/day, and monitoring is recommended 1
- In children with intellectual disability, extrapyramidal symptom profiles at therapeutic doses are comparable to placebo 9
