Caffeine and Restless Legs Syndrome Management

Understanding Restless Legs Syndrome

  • Restless Legs Syndrome (RLS) is characterized by an urge to move the legs usually accompanied by uncomfortable and unpleasant sensations in the legs and sometimes other body parts, with symptoms that are more pronounced in the evening or night, or may only occur during these times 1, 3
  • RLS symptoms are relieved with movement such as walking or stretching 1

Caffeine's Role in RLS

  • Caffeine can interfere with normal sleep patterns and exacerbate RLS symptoms as a stimulant 1, 3
  • The American College of Physicians and other guideline societies recommend avoiding caffeine for RLS management, as it is specifically mentioned in clinical guidelines as a substance to avoid 1, 3, 4

Management Recommendations

Non-pharmacological Approaches

  • Avoiding caffeine, alcohol, and nicotine too close to bedtime is recommended, along with increasing exposure to bright light during the day, engaging in regular exercise in the morning and/or afternoon, avoiding heavy meals or drinking within 3 hours of bedtime, and enhancing sleep environment (dark, quiet room, comfortable temperature) 1, 3, 4

Pharmacological Considerations

  • For patients with persistent RLS symptoms despite lifestyle modifications, dopaminergic agents are the first-line treatment for moderate to severe RLS, according to the American Geriatrics Society 5
