Safe PRN Medications for Anxiety with MAOIs

Introduction to Safe Options

  • The American Academy of Neuropsychopharmacology recommends benzodiazepines as the only truly safe PRN option for acute anxiety when taking MAOIs, due to their mechanism of action through GABA pathways and lack of interaction risk with MAOIs 1, 2
  • Benzodiazepines can be used as needed without washout periods or dose adjustments when combined with MAOI therapy, with common options including lorazepam, clonazepam, or alprazolam for breakthrough anxiety symptoms 1

Contraindicated and Cautionary Options

  • The FDA and medical guidelines contraindicate the use of serotonergic PRN medications, such as SSRIs, SNRIs, or serotonin precursors like tryptophan, with MAOIs due to the risk of life-threatening serotonin syndrome 3, 4
  • First-generation antihistamines like hydroxyzine may cause additive sedation and cognitive effects, particularly problematic in elderly patients, and are considered a poor choice for PRN use with MAOIs 2

Critical Safety Considerations

  • Any serotonergic PRN medication is absolutely contraindicated with MAOIs due to life-threatening serotonin syndrome risk, which presents with mental status changes, autonomic instability, and neuromuscular changes 3, 4
  • All over-the-counter cold medications containing pseudoephedrine, phenylephrine, or dextromethorphan must be avoided as they can trigger hypertensive crisis, and patients must be warned to avoid energy drinks, weight loss products, and stimulant-containing supplements 2

Clinical Algorithm and Patient Education

  • The clinical algorithm for PRN anxiety with MAOIs recommends prescribing short-acting benzodiazepines as the first-line option, and absolutely avoiding buspirone, any SSRI/SNRI, and all sympathomimetic agents 1, 2
  • Patient education is mandatory, providing written instructions listing all contraindicated medications, signs of hypertensive crisis, and the requirement to inform all providers about MAOI use 2, 4
