Evaluation and Management of Bilateral Peripheral Neuropathy

Initial Clinical Assessment

  • The American Diabetes Association recommends determining if numbness is accompanied by pain, burning, tingling, or pure numbness with loss of balance to assess small and large fiber involvement in patients with bilateral peripheral neuropathy 1, 2
  • The assessment of associated symptoms, such as autonomic dysfunction, motor weakness, and gait disturbance, is crucial in the initial clinical assessment of bilateral peripheral neuropathy 2

Neurological Examination

  • The American Academy of Neurology recommends performing systematic testing to characterize fiber type involvement, including small fiber function and large fiber function, in patients with bilateral peripheral neuropathy 3, 4
  • The assessment of small fiber function using pinprick and temperature sensation testing, and large fiber function using vibration perception and ankle reflexes, is essential in the neurological examination of bilateral peripheral neuropathy 1, 2, 3, 4

Treatment Approach

  • The American Diabetes Association recommends optimizing glucose control to prevent or delay neuropathy in patients with diabetes, and addressing underlying causes such as vitamin B12 deficiency and hypothyroidism 1, 2
  • The use of first-line agents, such as pregabalin or duloxetine, is recommended for the management of neuropathic pain in patients with bilateral peripheral neuropathy 1, 4

Follow-up and Monitoring

  • Regular reassessment of symptoms and examination findings is necessary to monitor progression and adjust treatment accordingly in patients with bilateral peripheral neuropathy 1
  • Annual foot examinations are mandatory if loss of protective sensation is documented in patients with bilateral peripheral neuropathy 3, 5
