Differential Diagnosis for Unilateral Foot Numbness and Pain

Introduction to Differential Diagnoses

  • The American College of Physicians suggests that normal EMG and NCS effectively exclude large fiber neuropathy, narrowing the differential diagnosis to conditions affecting small nerve fibers or non-neurologic etiologies 1, 2, 3.

Primary Differential Diagnoses

  • The American Diabetes Association recommends considering small fiber neuropathy in patients with burning pain, "electrical shock" sensations, and altered temperature perception, which are hallmark features of small C-fiber and Aδ-fiber dysfunction 1.
  • Patients with small fiber neuropathy characteristically describe "walking barefoot on marbles" or "walking barefoot on hot sand", and pain typically worsens at night, causing significant sleep disturbance 1.
  • The American Academy of Neurology states that standard nerve conduction studies will be normal in small fiber neuropathy because they only assess large myelinated fibers 1.
  • The European Federation of Neurological Societies recommends considering skin biopsy for intraepidermal nerve fiber density if small fiber neuropathy is suspected 3.
  • The American College of Physicians suggests screening for diabetes, vitamin B12 deficiency, and other metabolic causes in patients with small fiber neuropathy 1, 3.
  • The American Academy of Orthopaedic Surgeons recommends evaluating for back pain, positional symptom variation, and dermatomal distribution to assess for radiculopathy, which is suggested by unilateral distribution 1, 5.
  • The American Heart Association states that resting ankle-brachial index (ABI) ≤0.90 confirms PAD diagnosis, and if ABI >1.40, obtaining toe-brachial index with TBI <0.70 indicates PAD 5.
  • The European Society of Cardiology recommends examining for cool skin, bruits, elevation pallor, and dependent rubor to assess for PAD 5.
  • The International Diabetes Federation suggests that accurate differential diagnosis between osteomyelitis, soft tissue infection, and Charcot foot is crucial because these conditions require different treatments 6, 7.

Diagnostic Algorithms

  • The American College of Physicians recommends palpating all four pedal pulses bilaterally and performing resting ABI to exclude vascular disease 5.
  • The American Academy of Neurology suggests evaluating for back pain, positional symptom variation, and dermatomal distribution to assess for radiculopathy 1, 5.
  • The European Federation of Neurological Societies recommends assessing pinprick sensation and temperature perception to evaluate for small fiber neuropathy 2, 3.
  • The American Diabetes Association suggests screening for diabetes with HbA1c and fasting glucose to evaluate for small fiber neuropathy 1, 5.
  • The American College of Radiology recommends obtaining plain X-rays initially and considering MRI or nuclear medicine studies to differentiate Charcot foot from infection if diabetic 6, 7.

Common Pitfalls to Avoid

  • The American Academy of Neurology warns against assuming normal EMG/NCS excludes all neuropathy, as it only excludes large fiber involvement 1.
  • The American College of Physicians advises against overlooking the significance of unilateral presentation, which is atypical for metabolic neuropathies and should trigger evaluation for structural causes 1.
  • The American Heart Association recommends against relying solely on symptoms to diagnose PAD, as up to 30-60% of PAD patients are asymptomatic or have atypical leg symptoms 5.
