Diltiazem Dosing for Emergency Settings

Intravenous Diltiazem Administration

  • For rapid heart rate control in emergency settings, intravenous diltiazem should be administered as a 0.25 mg/kg bolus over 2 minutes, followed by either a continuous infusion of 5-15 mg/hour or transition to oral therapy, as recommended by the European Heart Journal 1, 2
  • Onset of action occurs within 2-7 minutes after intravenous administration of diltiazem 1
  • Following the initial bolus, start infusion at 5-15 mg/hour and titrate based on heart rate response and blood pressure, according to the European Heart Journal 1, 2
  • Maximum infusion rate typically does not exceed 15 mg/hour 1

Transition to Oral Diltiazem

  • For long-term maintenance, oral extended-release diltiazem may be administered at 120-360 mg daily in divided doses or as a single dose with extended-release formulations, as recommended by the European Heart Journal 1, 3
  • Maximum daily oral dose is 360 mg 1, 3

Monitoring and Precautions

  • Avoid diltiazem in patients with heart failure or impaired ventricular function (LVEF <40%), as contraindicated by the European Heart Journal 1, 2
  • Contraindicated in pre-excited atrial fibrillation or flutter (WPW syndrome) 1, 4
  • Avoid in patients with AV block greater than first degree without a pacemaker 3
  • Use with caution in combination with beta-blockers due to risk of bradyarrhythmias 3
  • Bradycardia may occur, especially with concomitant use of other negative chronotropic agents 4
  • Potential worsening of heart failure in predisposed patients 4
  • Have resuscitation equipment available during IV administration, and consider atropine or temporary pacing for severe bradycardia 4
  • Synchronized cardioversion is preferred over medication in hemodynamically unstable patients 4
