Epidural Anesthesia for Total Hysterectomy

Target Insision and Catheter Position

  • For transverse upper incisions, such as those in total hysterectomies, the epidural catheter should be inserted between the T5 and T8 roots to provide adequate analgesia 1
  • The epidural catheter tip should be placed at the T6-T8 level to provide adequate analgesia, as recommended by the American Society of Anesthesiologists 1

Catheter Insertion Depth

  • The insertion of the epidural catheter to a depth of 3-5 cm into the epidural space minimizes the risk of dislodgement while avoiding complications, according to the European Society of Anaesthesiology 2

Initial Dose Administration

  • No cited facts are available for this section

Target Dermatom and Viseratom Segments

  • The sensory block level for anesthesia should be tested (cold and pinprick) before induction of general anesthesia, as recommended by the American Society of Regional Anesthesia 1
  • The block height should be monitored every 5 minutes until no further extension is observed, with attention to signs of high block, such as agitation, significant hypotension, bradycardia, upper limb weakness, dyspnea, or difficulty speaking 1

Important Clinical Considerations

  • Epidural thoracic anesthesia is recommended based on data showing superior pain relief and fewer respiratory complications compared to intravenous opioids, according to the American College of Obstetricians and Gynecologists 1

Epidural Placement Guidelines

Introduction to Epidural Placement

  • The American Society of Anesthesiologists recommends targeting the thoracic epidural at levels T6-T10 for major abdominal surgery, lumbar epidural at L3/4 for hip replacement, and lumbar epidural at L3/4 for knee surgery 3

Epidural Placement Based on Procedure Type

  • For major abdominal surgery, the target sensory level for anesthesia is T4-T6 to ensure adequate analgesia 4
  • The European Society of Anesthesiology recommends using an initial dose of 0.2-0.3 ml/kg of 0.25% bupivacaine (maximum 10 ml) for thoracic epidural anesthesia in adults 5
  • For total hip replacement, a lumbar plexus block has higher efficacy compared to a femoral nerve block 3
  • The American Society of Regional Anesthesia recommends placing the lumbar epidural at L3/4 as the standard location for lower extremity procedures 6
  • For lumbar epidural anesthesia in adults, use an initial dose of 0.5 ml/kg of 0.25% bupivacaine (maximum 15 ml) 5

Monitoring and Safety

  • The American Society of Anesthesiologists recommends assessing block height every 5 minutes until no further extension is observed 4
  • Monitor signs of high block: increased agitation, significant hypotension, bradycardia, upper limb weakness, dyspnea, or difficulty speaking 4
