Management of Subtherapeutic Valproate Levels

Understanding Valproate Therapeutic Levels

  • Subtherapeutic valproate levels increase the risk of breakthrough seizures, which can significantly impact morbidity and mortality in patients with epilepsy, according to the American College of Emergency Physicians 1, 2

Step 1: Dose Adjustment

  • Administering Depakote ER as a once-daily dose, preferably at bedtime to minimize side effects like drowsiness, is recommended by Praxis Medical Insights 3

Step 3: Consider Alternative Approaches if Needed

  • If rapid seizure control is needed and oral therapy is insufficient, consider IV valproate loading at up to 30 mg/kg at a maximum rate of 10 mg/kg/min, as suggested by the American College of Emergency Physicians 4, 5
