Immediate Evaluation and Management of Suspected Eating Disorder with Diuretic Misuse

Critical Initial Assessment

  • The American Academy of Pediatrics recommends urgent medical evaluation with vital signs, orthostatic measurements, ECG, and comprehensive metabolic panel to assess for life-threatening electrolyte abnormalities and cardiac complications from diuretic abuse, followed by immediate referral to a multidisciplinary eating disorder team 1, 2
  • The discordance between a patient's self-report and a family member's concern, combined with an empty diuretic bottle, represents a classic presentation of eating disorder denial and high-risk purging behavior 1, 3

Emergency Laboratory Testing

  • The American Academy of Pediatrics recommends ordering a comprehensive metabolic panel to detect hypokalemia, hyponatremia, and hypochloremic alkalosis from diuretic abuse, as well as a complete blood count for anemia, leukopenia, and thrombocytopenia, and an ECG to assess QTc prolongation 2, 4

Hospitalization Criteria

  • The Academy for Eating Disorders recommends hospitalization if any of the following are present: heart rate <50 bpm, orthostatic vital sign changes, temperature <36°C, QTc prolongation on ECG, severe electrolyte abnormalities, weight <75% ideal body weight or BMI <16 kg/m², or inability to control purging behaviors 2, 5

Psychiatric Assessment

  • The American Psychiatric Association recommends screening for suicidality, comorbid depression, anxiety, obsessive-compulsive disorder, history of physical or sexual abuse, and degree of denial and insight into illness 1, 2, 4

Treatment Approach

  • The Academy for Eating Disorders recommends referring immediately to a multidisciplinary eating disorder team including a psychiatrist or psychologist, registered dietitian, and medical provider for ongoing monitoring, and using family-based treatment as first-line therapy for adolescents with involved caregivers 1, 2, 5
  • The American Academy of Pediatrics recommends hospitalizing for medical stabilization before specialized eating disorder treatment if medically unstable, and initiating slow, cautious refeeding with phosphorus supplementation to prevent fatal refeeding syndrome 2, 5
