Safety of Combining Trimethoprim/Sulfamethoxazole with Meropenem

Introduction to Combination Therapy

  • The combination of trimethoprim/sulfamethoxazole (TMP-SMX) and meropenem can be safely used in clinical practice, particularly for complex infections requiring broad-spectrum coverage, such as melioidosis, with no major contraindicated drug-drug interactions 1

Patient Monitoring and Special Considerations

  • The American Geriatrics Society recommends that TMP-SMX should be used with caution in patients with reduced kidney function, as it increases the risk of hyperkalemia, especially when combined with other medications 2
  • Elderly patients are at higher risk for adverse effects and should be monitored more closely when using the combination of TMP-SMX and meropenem 2
  • Patients taking other medications that can affect potassium levels should be monitored for hyperkalemia when TMP-SMX is added 2
  • Patients with hepatic impairment may require additional monitoring, particularly when using TMP-SMX 3

Clinical Applications of the Combination

  • The combination of meropenem and TMP-SMX may be used for the treatment of melioidosis, where meropenem is used for the intensive phase and TMP-SMX for the eradication phase 1
  • This combination can be considered for complex polymicrobial infections requiring broad-spectrum coverage, although the strength of evidence is not specified 4
