Management of Postmenopausal Spotting in Women on Estrogen Therapy

  • For women with an intact uterus on estrogen therapy, the American College of Cardiology recommends adding medroxyprogesterone acetate 10 mg orally daily for 10-14 days each month to provide endometrial protection 1
  • The American College of Cardiology also suggests an alternative regimen of medroxyprogesterone acetate 2.5 mg orally every day (continuous combined therapy) to minimize side effects while maintaining endometrial protection 1

Evaluation and Monitoring

  • The US Preventive Services Task Force (USPSTF) guidelines are not applicable to the management of spotting in postmenopausal women on estrogen therapy, as they pertain to the initiation of hormone therapy for prevention of chronic conditions 2, 3

Minimal Progesterone Dose for Endometrial Protection

Clinical Evidence

  • The American College of Physicians recommends that oral conjugated equine estrogen 0.625 mg/day combined with medroxyprogesterone acetate 2.5 mg/day provides adequate endometrial protection, with only 6% incidence of hyperplasia over 36 months compared to 64% with estrogen alone, based on evidence from the Women's Health Initiative 4, 5
  • The use of compounded bioidentical hormones should be avoided, as their safety and effectiveness for endometrial protection have not been established through proper drug approval processes, according to the American College of Obstetricians and Gynecologists 4, 5
