Dietary and Nutritional Treatment for Insulin Resistance

Introduction to Dietary Recommendations

  • The American Diabetes Association recommends that patients with insulin resistance follow a diet rich in complex carbohydrates with a low glycemic index (vegetables, fruits, legumes, whole grains) with high fiber content (14g/1000 kcal), limiting saturated fats to <7% of total calories, avoiding added sugars and sugary drinks, combined with a weight loss of 5-7% and 150 minutes of aerobic exercise per week 1, 2

Modificaciones Dietéticas Fundamentales

Carbohidratos

  • Prioritize carbohydrates with a low glycemic index: non-starchy vegetables, whole fruits (not juices), legumes, and whole grains with high fiber content 1
  • Consume 14g of fiber per 1000 kcal of total intake, emphasizing foods with higher fiber content 1, 3
  • Eliminate sugary drinks and minimize foods with added sugars to control weight and reduce cardiovascular risk 1

Grasas

  • Limit saturated fats to <7% of total calories 1, 3
  • Minimize trans fats completely 3
  • Emphasize a Mediterranean-style diet rich in monounsaturated and polyunsaturated fats to improve glucose metabolism and reduce cardiovascular risk 1

Patrones Dietéticos Recomendados

  • The Mediterranean diet improves glucose metabolism, reduces insulin resistance, and cardiovascular risk 1

Pérdida de Peso

  • Aim for a weight loss of 5-7% of initial body weight for patients with overweight or obesity 4, 2
  • Create a calorie deficit of 500-1000 calories per day from maintenance requirements 2

Ejercicio Físico

  • Perform at least 150 minutes of moderate-intensity aerobic exercise per week, distributed over at least 3 days with no more than 2 consecutive days without activity 1, 4, 2
  • Include resistance exercise 2-3 times a week on non-consecutive days, involving major muscle groups 4, 2

Suplementación

  • Do not recommend vitamin and mineral supplements without underlying deficiencies 1, 5
  • Do not recommend antioxidant supplements (vitamins E, C, carotene) due to lack of efficacy and safety concerns 1, 3, 6

Otros Componentes Nutricionales

  • Limit sodium intake to <2,300 mg/day, with greater restriction if concomitant hypertension is present 1
  • If alcohol is consumed, limit to 1 drink/day for women and 2 drinks/day for men 1, 5

Estrategias de Implementación

  • Maintain consistency in meal times and amounts if using medications that increase insulin 1, 2
  • Reevaluate regimen every 3-6 months and adjust as needed 4
  • Use glucose monitoring to determine if food adjustments are sufficient or if medication is needed 7, 6
