Heparin DVT Prophylaxis Dosing

Standard Prophylactic Dosing

  • The American College of Chest Physicians recommends UFH 5000 units subcutaneously every 8 hours for standard-weight patients, providing a more consistent anticoagulant effect compared to twice-daily administration 1

Special Population Considerations

  • For cancer patients requiring VTE prophylaxis, the National Comprehensive Cancer Network endorses UFH 5000 units subcutaneously every 8 hours as the standard regimen 1
  • UFH is the preferred agent for patients with severe renal insufficiency, with the standard 5000 units every 8 hours dosing used, as dose adjustment is not required 1, 2, 3
  • For elderly trauma patients, if UFH is used instead of enoxaparin, the dose is 5000 units every 8 hours, with LMWH being superior to UFH in preventing DVT 3

Duration and Timing

  • Prophylaxis should be initiated 2 hours before surgery for surgical patients 1
  • Continue prophylaxis for at least 7-10 days postoperatively, or until the patient is fully ambulatory 1
  • Medical patients should receive prophylaxis until fully ambulatory or hospital discharge 1

Clinical Pitfalls to Avoid

  • Therapeutic UFH requires weight-based dosing with aPTT monitoring, and should not be confused with prophylactic dosing 2, 4
  • The American College of Cardiology is not mentioned, however, the timing with neuraxial anesthesia is critical, and administering anticoagulants too close to spinal or epidural anesthesia increases the risk of spinal hematoma 1

Comparison with LMWH

  • LMWH is generally preferred over UFH for DVT prophylaxis when not contraindicated, with superior efficacy and lower rates of complications 3, 5, 6
  • UFH remains the appropriate choice for patients with severe renal impairment, those at very high bleeding risk, or when rapid reversibility is needed 1, 2
