Therapeutic Options for Adjustment Disorder
Psychotherapy Options
- The American College of Cardiology is not applicable here, however, Individual CBT is the most evidence-based psychological intervention for adjustment disorder, focusing on modifying cognition and behavior to reduce unpleasant feelings and improve social adjustment 1
- Self-help with support based on CBT principles is recommended for patients who do not want face-to-face therapy 2
- For mild adjustment disorder, psychotherapy alone is typically sufficient without medication 1
- Family involvement should be incorporated whenever possible, particularly when treating children or adolescents with adjustment disorder 3
- Problem-solving treatment for those with depressive symptoms is an evidence-based approach 4
- Relaxation training as an adjunctive intervention is also recommended 4
Pharmacological Options
- For moderate to severe adjustment disorder with significant anxiety or depression symptoms, pharmacotherapy may be considered in addition to psychotherapy 1
- Medication options include anxiolytics, such as benzodiazepines, for short-term management of severe anxiety symptoms 1, 5
- Antidepressants, particularly SSRIs, are recommended for adjustment disorder with depressive features 1, 5
Treatment Algorithm
- Initial assessment and diagnosis should include confirmation of adjustment disorder diagnosis with identifiable stressor and assessment of severity of symptoms and suicide risk 1
- Evaluation for comorbid conditions is also necessary 1
- For mild adjustment disorder, psychotherapy alone, with CBT preferred, is recommended 1
- For moderate to severe adjustment disorder, a combined approach with psychotherapy and medication may be considered 1
- Regular assessment of symptom improvement, evaluation of treatment adherence, and adjustment of treatment plan based on response are crucial 1
