Radiotherapy in Cancer Treatment

Current Role of Radiotherapy

  • The British Journal of Cancer states that radiotherapy can be delivered as definitive treatment with curative intent, adjuvant therapy following surgery, or palliative treatment for symptom control 1
  • Modern techniques include three-dimensional conformal RT, intensity-modulated RT (IMRT), and image-guided RT, offering improved precision in dose delivery, as reported by the British Journal of Cancer and Praxis Medical Insights 1, 2

Clinical Applications in Specific Cancers

Colorectal Cancer

  • The Annals of Oncology reports that radiotherapy significantly impacts survival in resectable rectal tumors and can facilitate R0 resection in initially unresectable cases, with preoperative radiotherapy reducing local recurrence rates and providing long-term prevention of recurrence 3, 4, 5
  • Short-course preoperative radiotherapy (5×5 Gy) has shown effectiveness comparable to protracted schedules (45-50 Gy over 4-5 weeks), according to the Annals of Oncology 5

Non-Small Cell Lung Cancer (NSCLC)

  • The Annals of Oncology states that radiotherapy provides rapid symptom control for pain, hemoptysis, cough, dyspnea, superior vena cava syndrome, and spinal cord compression, with stereotactic body radiotherapy (SBRT) or high-dose fractionated irradiation combined with systemic treatment showing promising results for oligometastatic disease 6, 7
  • For solitary brain metastasis, resection or stereotactic radiosurgery (SRS) are primary treatment options, with whole brain radiotherapy (WBRT) improving local control but not overall survival, as reported by the Annals of Oncology 6

Renal Cell Carcinoma (RCC)

  • The Annals of Oncology reports that modern high-precision radiotherapy methods such as SBRT can overcome the apparent radioresistance of RCC, with radiotherapy effectively palliating local and symptomatic metastatic RCC disease, particularly in critical sites like bones and brain 8, 9
  • For symptomatic bone metastasis, local radiotherapy provides good symptom relief in up to two-thirds of cases with complete symptomatic responses in 20-25% of patients, according to the Annals of Oncology 8, 9

Prostate Cancer

  • Praxis Medical Insights reports that for metastatic prostate cancer, a single 800 cGy fraction is as effective as 3000 cGy in 10 fractions for non-vertebral metastases, with radiopharmaceuticals like radium-223 significantly improving overall survival and prolonging time to first skeletal-related event in patients with symptomatic bone metastases 10

Future Prospects and Advances

Technological Innovations

  • The British Journal of Cancer and Praxis Medical Insights state that advances in imaging and therapy beam precision have greatly improved the therapeutic ratio and accuracy of modern radiotherapy, with newer techniques such as IMRT and image-guided RT offering the possibility of increasing the dose to the tumor while sparing healthy tissue 1, 2
  • Stereotactic body RT techniques are expanding for treating oligometastatic disease with precisely delivered RT fields and ablative doses, as reported by the British Journal of Cancer 1
