Denosumab and Hypercalcemia

Hypercalcemia Risk Profile

  • Denosumab decreases calcium levels in patients with hypercalcemia of malignancy, and is used therapeutically for this indication, as recommended by the American Society of Clinical Oncology 1, 2

Mechanism of Rebound Hypercalcemia

  • Denosumab's effect on bone turnover is rapidly reversible after cessation, unlike bisphosphonates, which can lead to a marked rebound increase in bone resorption, as reported by the European Society for Medical Oncology 3, 4

Prevention Strategies

  • Transition to bisphosphonate therapy after 5 years of denosumab treatment in adults can reduce or prevent rebound bone turnover, as suggested by the National Comprehensive Cancer Network 3, 4
  • Consider continuation of denosumab rather than discontinuation in patients at high risk, such as those with primary hyperparathyroidism or metastatic bone disease, as recommended by the Endocrine Society 3, 4

Critical Caveats

  • The rebound effect is associated with a marked increase in vertebral fracture risk in addition to hypercalcemia, highlighting the need for careful monitoring and management, as emphasized by the American College of Rheumatology 3, 4
