ESA Dosing for Dialysis Patients with Anemia of CKD
Initial Dosing Considerations
- For adult CKD patients on dialysis, ESA therapy should be initiated when hemoglobin is less than 10 g/dL, according to the Kidney International guidelines 1
- Alternatively, if using epoetin alfa, the recommended starting dose is 50-100 IU/kg three times weekly or 10,000-40,000 units weekly, as suggested by Blood advances 2, 3
- For patients transitioning from hospital to SNF who were previously on an ESA, it's essential to restart therapy promptly to prevent further hemoglobin decline, as recommended by Kidney International 1
Target Hemoglobin Range
- The target hemoglobin for dialysis patients should generally be between 10-11 g/dL, according to Kidney International 1, 4
- ESA therapy should be used to avoid having hemoglobin fall below 9.0 g/dL, as suggested by Kidney International 1
- KDIGO guidelines recommend against using ESAs to maintain hemoglobin concentration above 11.5 g/dL in adult patients with CKD, as stated in Kidney International 1
Monitoring and Dose Adjustments
- For darbepoetin alfa, increase dose by 25% if hemoglobin increases by less than 1 g/dL after 4 weeks of therapy, as recommended by Blood advances 3
- Decrease dose by 25% when hemoglobin reaches a level needed to avoid transfusion or if hemoglobin increases more than 1 g/dL in 2 weeks, according to Blood advances 2, 3
- If hemoglobin exceeds the target range, reduce dose rather than withholding ESA completely to avoid hemoglobin cycling, as suggested by the American Journal of Kidney Diseases and Kidney International Supplements 5, 6
Important Considerations and Precautions
- Be cautious with ESA therapy in patients with active malignancy, history of stroke, or history of malignancy, as recommended by Kidney International 1
