Lemborexant and Melatonin Combination for Insomnia Treatment
Introduction to Melatonin's Limited Role
- The American Academy of Sleep Medicine recommends against using melatonin as a treatment for sleep onset or sleep maintenance insomnia in adults, due to weak evidence showing only a 9-minute reduction in sleep latency compared to placebo 1, 2, 3
- Melatonin has minimal clinical benefit in chronic insomnia, with only small improvements in quality of sleep 4, 5
Safety and Efficacy Considerations
- The American Academy of Sleep Medicine notes that combination therapy data is generally lacking for insomnia medications, including lemborexant and melatonin 6, 7
- Melatonin is generally well-tolerated with minimal adverse effects reported in clinical trials 4, 5
Clinical Recommendations
- If a patient is already taking melatonin and requires additional insomnia treatment, consider discontinuing melatonin given its lack of evidence-based benefit for chronic insomnia 1, 2, 3
- Cognitive-behavioral therapy for insomnia (CBT-I) should be offered as first-line treatment when available 11
- All pharmacologic insomnia treatments, including lemborexant, carry weak strength of evidence by GRADE methodology and should be used short-term when possible 11
