Comprehensive Dementia Workup and Treatment

Diagnostic Workup

  • The American Academy of Neurology recommends using standardized cognitive screening tools to document cognitive impairment, including Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA) 1, 2
  • A comprehensive dementia workup should include cognitive assessment, laboratory testing for reversible causes, neuroimaging, and treatment with both non-pharmacological approaches and appropriate medications based on dementia severity 1, 3, 2
  • Informant assessment should be obtained from a reliable informant about changes in cognition, function, and behavior, using tools like AD8, IQCODE, or Quick Dementia Rating System 1, 3, 4
  • Core laboratory investigations should include complete blood count, comprehensive metabolic panel, thyroid function tests, vitamin B12 and folate levels, and liver function tests 2, 5
  • Anatomical neuroimaging is recommended in most situations, with MRI preferred over CT, especially for detecting vascular lesions 2

Treatment Approaches

  • The American Geriatrics Society recommends cholinesterase inhibitors (donepezil, rivastigmine, galantamine) for mild to moderate dementia, and memantine for moderate to severe dementia 6
  • Physical activity interventions, including aerobic exercise and/or resistance training, should be considered for patients with dementia 7, 8, 9
  • Dietary recommendations include a Mediterranean diet, high consumption of mono- and polyunsaturated fatty acids, and low consumption of saturated fatty acids 10, 7, 8, 9

Follow-up and Monitoring

  • The Alzheimer's Association recommends tracking response to treatment using a multi-dimensional approach, including assessment of cognition, functional autonomy, behavioral symptoms, and caregiver burden 3, 2
  • Follow-up should be every 6-12 months, with more frequent assessment for patients with behavioral symptoms 2

Special Considerations

  • For patients with subjective cognitive decline but normal cognitive testing, a comprehensive diagnostic workup should be conducted to identify reversible causes, and annual follow-ups should be considered if corroborative history is positive 2, 4
  • For end-of-life care, focus on improving quality of life and maximizing comfort, and consider palliative care for terminal stages 2, 6

Optimal Treatment Plan for Dementia

Comprehensive Assessment

  • Assessment of hearing function is essential, as hearing loss is associated with cognitive decline, and treatment of hearing loss may improve cognition 11, 12
  • Sleep evaluation should include assessment for sleep apnea, which when treated may improve cognition, and targeting 7-8 hours of sleep per night and avoiding severe sleep deprivation may also improve cognition 11, 13

Non-Pharmacological Interventions

  • Exercise is recommended for all people living with dementia, though specific duration and intensity cannot be specified, and engagement in cognitively stimulating activities should be encouraged 11, 12, 14
  • Group cognitive stimulation therapy should be considered for mild to moderate dementia, providing activities that stimulate thinking, concentration, and memory in a social setting 13, 14
  • Social engagement opportunities should be supported throughout the life course 11, 12

Pharmacological Management

  • Medications with anticholinergic properties should be minimized or avoided, with alternative medications used when possible, and regular medication review should be conducted to identify and minimize potentially inappropriate medications 11, 14

Risk Factor Modification

  • Hearing loss should be identified and treated with appropriate audiologic rehabilitation, and interventions to manage frailty should be implemented to reduce dementia burden 11, 12, 14
  • Educational attainment and ongoing educational experiences should be supported, and multidimensional health assessment should be performed to identify reversible conditions 11, 12, 14
