Etiology and Molecular Mechanisms of Mantle Cell Lymphoma
Primary Molecular Mechanism
- The chromosomal translocation t(11;14)(q13;q32) is the molecular hallmark of mantle cell lymphoma, leading to constitutive overexpression of cyclin D1 protein, according to the National Comprehensive Cancer Network 1, 2
- The t(11;14) translocation can be detected by FISH analysis or cytogenetics, demonstrating the juxtaposition of the CCND1 locus with the IgH locus, as recommended by the National Comprehensive Cancer Network 1, 3
Variant Molecular Pathways
- Overexpression of cyclin D2 or cyclin D3 may substitute for cyclin D1, maintaining the same dysregulated cell cycle machinery in rare cyclin D1-negative cases, according to the National Comprehensive Cancer Network 1, 4
- CCND2 gene rearrangements occur in approximately 55% of cyclin D1-negative cases, associated with high cyclin D2 mRNA expression, as reported by the National Comprehensive Cancer Network 4, 5
- These cyclin D1-negative variants demonstrate similar genomic signatures and clinical behavior to cyclin D1-positive cases, according to the National Comprehensive Cancer Network 4, 6
Secondary Molecular Alterations
- Nuclear overexpression of the transcription factor SOX11 is observed in almost all mantle cell lymphoma cases, regardless of cyclin D1 expression level, as noted by the National Comprehensive Cancer Network 1, 4
Epidemiologic Risk Factors
- Male predominance with a 3:1 male-to-female ratio is observed in mantle cell lymphoma, according to the Annals of Oncology 2, 7
- The annual incidence of mantle cell lymphoma has increased to 1-2 per 100,000 in Western populations, as reported by the Annals of Oncology 2, 7
Clinical Pitfalls
- MCL in situ represents the presence of cyclin D1-positive B-cells restricted to mantle zones in otherwise reactive lymph nodes, which has uncertain malignant potential and very indolent behavior, and should not be diagnosed as overt MCL, as recommended by the World Health Organization and the National Comprehensive Cancer Network 3, 4, 8
Pathogenesis of Mantle Cell Lymphoma
Molecular Mechanisms
- The European Society for Medical Oncology notes that dysregulation of the cell cycle machinery through uncontrolled G1/S phase progression contributes to tumor progression, with a strength of evidence based on molecular studies 9
- The development of mantle cell lymphoma progresses from early to classical and transformed forms, with secondary molecular alterations including ATM and CHK2 mutations, which occur in early stages of the disease 9
- Complex karyotypes develop during the course of the disease, with additional alterations in ATM and CHK2 accumulating, leading to aggressive variants 9
- INK4A/CDK4/RB1 pathway alterations characterize aggressive variants, with ARF/MDM2/p53 pathway disturbances leading to highly proliferative forms 9
