Precautions When Using Bipolar Cautery Near the Obturator Nerve

  • The American Society of Anesthesiologists recommends using short, intermittent bursts at the lowest feasible energy levels while maintaining direct visualization of the target area to minimize the risk of nerve stimulation and injury 1, 2, 3

Procedural Techniques

  • The American Academy of Otolaryngology-Head and Neck Surgery suggests maintaining direct visualization of the target bleeding site to prevent excessive tissue injury and increase chances of success 4, 5, 6
  • The American Society of Anesthesiologists advises using short, intermittent, and irregular bursts at the lowest feasible energy levels 1, 3, 7
  • The American Society of Anesthesiologists recommends positioning the patient and equipment to ensure the current pathway does not pass through or near sensitive neural structures 1, 3
  • The American Society of Anesthesiologists suggests considering using an ultrasonic (harmonic) scalpel as an alternative when working extremely close to the nerve 1, 2, 8

Special Situations

Patients with Implanted Electronic Devices

  • The American Society of Anesthesiologists recommends that for patients with cardiac rhythm management devices (pacemakers/ICDs), additional precautions are necessary, including positioning the patient so the current pathway does not pass through or near the device system 1, 2, 3
  • The American Society of Anesthesiologists advises considering temporarily reprogramming pacing functions or disabling rate-responsive functions 2
  • The American Society of Anesthesiologists suggests having temporary pacing and defibrillation equipment immediately available 7
