Vancomycin Dosing and Administration Guidelines
Introduction to Vancomycin Therapy
- The American College of Clinical Pharmacology recommends vancomycin dosing of 15-20 mg/kg (based on actual body weight) every 8-12 hours intravenously for adult patients with normal renal function 1
Dosing Considerations
- For patients with severe or serious infections (bacteremia, endocarditis, meningitis, pneumonia, osteomyelitis), a loading dose of 25-30 mg/kg (based on actual body weight) is recommended to rapidly achieve therapeutic concentrations 2, 3
- The maximum dose per administration should not exceed 2 grams 2
Pediatric Dosing
- For children, the recommended dose is 10 mg/kg per dose every 6 hours 1
Administration Guidelines
- The infusion rate should not exceed 10 mg/minute 3
- The infusion duration should be at least 60 minutes for each dose, choosing the longer of 60 minutes or the rate of 10 mg/minute 3
- The concentration of the solution should not exceed 5 mg/mL to minimize infusion-related reactions 3
Monitoring and Adjustment
- For severe infections, the target trough concentration is 15-20 μg/mL 1, 2
- For mild to moderate infections, the target trough concentration is 10-15 μg/mL 2
- The best pharmacodynamic parameter is an AUC/MIC ratio >400 2
- Trough samples should be taken at steady state, before the fourth or fifth dose 2
- Monitoring is mandatory for patients with renal dysfunction, obesity, or changes in volume of distribution 2, 3
Common Mistakes to Avoid
- Avoid using a fixed dose of 1 gram without considering the patient's weight, as this can lead to underdosing in most patients, especially those weighing >70 kg 3
- Avoid targeting high trough levels (15-20 μg/mL) for non-severe infections, as this increases the risk of nephrotoxicity without additional benefits 3
- If the vancomycin MIC is ≥2 μg/mL, consider alternative therapies (daptomycin, linezolid, ceftaroline) because the target AUC/MIC ratio may not be achievable 2, 3
