Fentanyl Dosing for Intubation

Dosage by Population

  • The American Academy of Pediatrics recommends a dose of 1-5 μg/kg IV for intubation in adults, with higher doses (3-5 μg/kg) preferred for patients with brain injury or multiple trauma, and reduced doses (2 μg/kg) for normotensive patients 1
  • For adult patients, the initial dose is 50-100 μg IV as a bolus, administered over 1-2 minutes, with supplemental doses of 25 μg every 2-5 minutes until adequate sedation is achieved 2
  • The American Society of Anesthesiologists recommends reducing the dose by 50% or more in patients over 60 years old or with ASA III or higher, to 25-50 μg as an initial bolus 2, 3, 4

Pharmacological Considerations

  • The onset of action is 1-2 minutes, with a duration of effect of 30-60 minutes 2, 3, 5
  • The American Heart Association warns that respiratory depression can last longer than the analgesic effect 5, 3, 4

Protocol for Rapid Sequence Intubation (RSI)

  • The American College of Emergency Physicians recommends combining fentanilo with etomidato and succinilcolina for RSI 1

Adverse Effects and Precautions

  • The National Institute of Health warns that respiratory depression is the primary adverse effect, which can last longer than the analgesic effect 5, 3, 4
  • The American Academy of Pediatrics notes that chest wall rigidity can occur with doses as low as 1 μg/kg with rapid administration, and is more common with high doses 1, 4
  • Reversal with naloxone is recommended, with a dose of 0.1-0.2 mg/kg IV, repeated every 2-3 minutes if necessary 1, 5, 3, 4

Mandatory Monitoring

  • The American Society of Anesthesiologists recommends continuous monitoring of oxygen saturation, blood pressure, and heart rate 5, 4, 3
  • Preparation for airway management is essential, as combination with other sedatives can cause significant respiratory depression 3, 4

Fentanyl Dosing and Administration for Intubation

Standard Dosing Protocol

  • The American Society of Anesthesiologists recommends administering fentanyl 0.5-1 μg/kg IV as a bolus, with subsequent doses of 0.5 μg/kg as required, given 2-5 minutes before intubation for optimal effect 6
  • For adult dosing, an initial bolus of 0.5-1 μg/kg IV is recommended, equivalent to 50-100 μg for an average adult, with supplemental doses of 0.5 μg/kg or 25 μg every 2-5 minutes until adequate effect 6
  • The optimal timing for administration is 2-5 minutes before intubation to achieve optimal hemodynamic blunting 6

Preparation and Dilution

  • Fentanyl typically comes as 50 μg/mL in 2 mL or 10 mL ampoules, and can be given undiluted or diluted in 5-10 mL normal saline for easier titration 6

Drug Interactions Requiring Dose Modification

  • When using concomitant opioids during awake intubation, caution is advised with doses >1.5 μg/mL 6
