Management of Osteopenia with Low Fracture Risk

Understanding Your Bone Density Results

  • The National Osteoporosis Foundation recommends treatment thresholds of ≥20% for major osteoporotic fracture and ≥3% for hip fracture, which are not met in this case with a 10-year probability of fracture of 7.0% for major osteoporotic fracture and 0.3% for hip fracture 1, 3
  • A 10-year probability of fracture of 7.0% for major osteoporotic fracture and 0.3% for hip fracture indicates a low fracture risk 1, 3
  • The decrease in bone mineral density since 2009 (-12.4% in left hip, -0.7% in lumbar spine) indicates progressive bone loss that should be monitored 1, 4
  • The American College of Rheumatology recommends optimizing calcium intake through diet and/or supplements (1,000-1,200 mg daily) 5
  • Ensuring adequate vitamin D intake (600-800 IU daily, with a target serum level ≥20 ng/ml) is recommended 5
  • Regular weight-bearing and resistance training exercises at least 3 times per week are recommended 2
  • Maintaining weight in the recommended range, quitting smoking, limiting alcohol consumption to 1-2 drinks per day, and following fall prevention strategies if at risk for falls are recommended lifestyle modifications 1, 5, 6

Monitoring Recommendations

  • Repeat bone density testing (DXA scan) in 2 years to assess progression is recommended 1, 4
  • Consider earlier reassessment (in 1 year) if new risk factors develop 4
  • Regular clinical assessment of fracture risk factors is recommended 5, 6

When to Consider Pharmacological Treatment

  • Pharmacological treatment would be indicated if the FRAX score increases to ≥20% for major osteoporotic fracture or ≥3% for hip fracture 1, 3
  • A fragility fracture, bone density decrease to the osteoporosis range (T-score ≤ -2.5), or development of additional significant risk factors would also indicate pharmacological treatment 3, 7
