Treatment of Anal Fissure
Introduction to Anal Fissure Management
- The American College of Surgeons recommends starting with conservative management including increased fiber (25-30g daily), adequate water intake, and warm sitz baths for acute anal fissures, which achieves a healing rate of 95% after 6 weeks of treatment 1, 2
First-Line Conservative Management
- Dietary and lifestyle modifications are strongly recommended as initial treatment for all acute anal fissures, with approximately 50% of acute anal fissures healing within 10-14 days with these conservative measures alone 1, 2
- Fiber supplementation of 25-30g daily is recommended to soften stools and minimize anal trauma during defecation 4
Pain Control Strategy
- Topical lidocaine can be applied as needed for local anesthesia, and standard oral analgesics may be added if topical agents are insufficient 1, 2, 5
Pharmacologic Treatment
- Compounded 0.3% nifedipine with 1.5% lidocaine cream can be applied three times daily for at least 6 weeks, with a healing rate of 95% after 6 weeks of treatment 3, 5
- The nifedipine blocks calcium channels in vascular smooth muscle, reducing internal anal sphincter tone and increasing local blood flow to the ischemic ulcer, while lidocaine provides local anesthesia to break the pain-spasm-ischemia cycle 3, 4, 5
Surgical Treatment Indications
- Surgery should be reserved for chronic fissures that fail to respond after 8 weeks of comprehensive non-operative management, with lateral internal sphincterotomy (LIS) remaining the gold standard with >95% healing rates and 1-3% recurrence 1, 2, 4, 5
