Aripiprazole Augmentation in Psychiatric Treatment

Dosing and Efficacy

  • The 2025 INTEGRATE schizophrenia guidelines suggest aripiprazole starting doses of 5 mg daily for psychiatric conditions, though this is in a different context 1
  • Aripiprazole has lower risk of extrapyramidal symptoms compared to other antipsychotics 1

Safety and Tolerability

  • Monitor for akathisia and restlessness, particularly in the first few weeks 1
  • Consider potential drug-drug interactions via cytochrome P450 2D6 and 3A4 pathways 2
  • Aripiprazole may cause headache, agitation, anxiety, insomnia, dizziness, or drowsiness 2
  • Reduce dose in elderly patients and poor metabolizers of CYP2D6 2

Precautions and Interactions

  • When combining aripiprazole with sertraline, be aware of serotonin syndrome risk, though aripiprazole's partial D2 agonism provides some theoretical protection, and monitor for symptoms including agitation, confusion, tremor, and autonomic instability, especially in the first 24-48 hours after dose changes 3
