Management of Severe Anemia in Pregnancy

Diagnosis and Assessment

  • The Centers for Disease Control and Prevention recommends a presumptive diagnosis of iron deficiency anemia can be made and treatment begun immediately in pregnant women who are not acutely ill 2

Treatment Algorithm for Severe Anemia

  • The American College of Physicians recommends prescribing 60-120 mg of elemental iron daily for severe anemia 1, 2
  • The Centers for Disease Control and Prevention suggests counseling pregnant women about iron-rich foods and dietary factors that enhance iron absorption 2
  • The Centers for Disease Control and Prevention recommends monitoring response with repeat hemoglobin/hematocrit testing after 4 weeks of treatment 2

Special Considerations

  • The American College of Physicians recommends that before switching to IV iron, confirm compliance with oral iron and rule out other causes of anemia unresponsive to iron therapy 2
  • The American College of Physicians suggests that for vegetarian women, iron requirements may be almost twice as high due to lower absorption of non-heme iron; consider higher supplementation doses 1, 4

Postpartum Management

  • The Centers for Disease Control and Prevention recommends monitoring hemoglobin levels at 4-6 weeks postpartum in women at risk for persistent anemia 2

Potential Complications and Prevention

  • The American College of Physicians recommends continuing iron supplementation throughout pregnancy and the postpartum period to prevent recurrence of anemia 1, 2
