Albumin Dosage for Children with Liver Cirrhosis

Indications and Specific Dosages

  • The American Association for the Study of Liver Diseases recommends a dosage of 6-8 g of albumin per liter of ascites fluid removed during large-volume paracentesis (>5 liters) 1, 2
  • The International Ascites Club suggests that albumin should be administered after completion of the paracentesis procedure 3
  • For patients with hepatorenal syndrome, the initial dosage is 1 g/kg (up to 100 g) on the first day, followed by 20-40 g/day in subsequent days, in combination with vasoconstrictor therapy 3
  • The European Association for the Study of the Liver recommends that the total dosage should not exceed 87.5-100 g during the initial loading phase due to the risk of fluid overload 3

Special Considerations for Children

  • The American Academy of Pediatrics suggests that some children may not require regular albumin infusions, while others may need them to prevent clinical consequences of decreased circulating volume and malnutrition 4, 5
  • The dosage for severe cases can be up to 1-4 g/kg/day, but should be reduced or discontinued in stable patients or those with progressing chronic kidney disease 4, 5

Contraindications and Important Warnings

  • The American Association for the Study of Liver Diseases advises against using albumin in cases of simple ascites without acute kidney injury, SBP, or large-volume paracentesis 3
  • The International Ascites Club recommends careful monitoring of hemodynamic responses and adherence to standard precautions to prevent fluid overload 3

Strongest Evidence

  • The strongest evidence supports the use of albumin for hepatorenal syndrome stage 2 or higher, SBP with elevated creatinine or bilirubin, and large-volume paracentesis (>5 liters) 3
