Differential Diagnoses for Young Girls Presenting with SLE-Like Features

Autoimmune and Rheumatologic Differentials

  • The American College of Rheumatology suggests that antiphospholipid antibody syndrome can present with multisystem involvement, including thrombotic cerebrovascular disease, which is common in SLE patients but may occur independently 1
  • The European League Against Rheumatism recommends testing for antiphospholipid antibodies in all suspected cases, as this determines anticoagulation strategy rather than immunosuppression alone 1

Immune Dysregulation Syndromes

  • The American Academy of Allergy, Asthma, and Immunology states that Autoimmune Lymphoproliferative Syndrome (ALPS) presents with lymphadenopathy, hepatosplenomegaly, and autoimmune cytopenias that can mimic SLE 2
  • The American Academy of Pediatrics recommends considering ALPS when lymphoproliferation and autoimmunity are prominent features 2

Neurological Differentials

  • The American Academy of Neurology suggests that Multiple Sclerosis can present with multifocal neurological deficits, particularly in young women, overlapping with neuropsychiatric SLE 3
  • The American Academy of Neurology recommends distinguishing Multiple Sclerosis by MRI showing periventricular white matter lesions with specific distribution patterns and CSF oligoclonal bands 3

Infectious Mimickers

  • The Centers for Disease Control and Prevention recommends serological testing for HTLV1 in endemic areas or at-risk populations 3
  • The Infectious Diseases Society of America suggests that Lyme disease can mimic SLE with multisystem involvement, particularly neurological and arthritic manifestations 3

Critical Diagnostic Approach

  • The American College of Rheumatology recommends ANA testing (mandatory entry criterion for SLE classification) with 1:160 dilution cutoff 4
  • The European League Against Rheumatism suggests that if ANA positive, further testing should include anti-dsDNA, anti-Smith, complement levels, complete blood count, comprehensive metabolic panel, urinalysis with microscopy 4

Differential Diagnoses for Systemic Lupus Erythematosus

Autoimmune and Rheumatologic Conditions

  • The American College of Rheumatology suggests testing for persistent moderate-to-high anticardiolipin, anti-β2-glycoprotein IgG/IgM, or lupus anticoagulant to differentiate Antiphospholipid Antibody Syndrome from SLE 5

Infectious Mimickers

  • The European League Against Rheumatism recommends considering Subacute Bacterial Endocarditis in patients with fever, arthritis, glomerulonephritis, and positive ANA, and notes that significantly elevated CRP (>50 mg/L) should prompt exclusion of infection in suspected SLE 6
  • The American College of Rheumatology suggests that blood cultures and echocardiography are essential in diagnosing Subacute Bacterial Endocarditis, which can mimic SLE 6

Neurological Conditions

  • The American Academy of Neurology notes that MRI showing periventricular white matter lesions with specific distribution patterns and CSF oligoclonal bands can help distinguish Multiple Sclerosis from neuropsychiatric SLE, and that the average sensitivity of MRI in active NPSLE is only 57% 5

Hematologic Malignancies

  • The American College of Rheumatology reports that SLE patients have an increased risk for non-Hodgkin's lymphoma, which can present with cytopenias, lymphadenopathy, and constitutional symptoms 7, 8

Dermatologic Mimics

  • The European League Against Rheumatism suggests that many conditions mimic lupus-specific cutaneous lesions and require evaluation by an experienced dermatologist with skin biopsy for histological analysis, and that repeat biopsy is recommended if lesion morphology changes or treatment fails 6, 9
