Buspirone Dosing Guidelines for Adult Anxiety

Initial Dosing and Titration

  • The American Academy of Family Physicians recommends starting with 5 mg twice daily (BID) for most adult patients with generalized anxiety disorder 1
  • The alternative starting approach is 7.5 mg twice daily, though the lower 5 mg BID dose is more commonly recommended in guidelines 1
  • Increase dosage gradually by increments of 5 mg every 2-3 days as tolerated to reach the therapeutic range 1

Special Population Considerations

  • Elderly or debilitated patients should start at lower doses (2.5-5 mg twice daily) and titrate more gradually, as this population may be more sensitive to medication effects 1

Critical Timing Considerations

  • Full therapeutic trial requires 4-8 weeks before determining efficacy 1

Target Dosing

  • Target therapeutic dose is 15-30 mg/day, with a maximum recommended dose of 60 mg/day (20 mg three times daily) 1

Buspirone: Onset of Effect and Indicated Use

Onset of Therapeutic Effect

  • Buspirone typically requires 2–4 weeks of continuous dosing before patients notice anxiolytic benefits, making early counseling essential to maintain adherence. 2

Indicated Patient Population

  • The medication is appropriate for individuals with mild to moderate anxiety or agitation and is not recommended for acute anxiety episodes that require rapid symptom control. 2
