Guideline for Tapering Oxcarbazepine in Long‑Term Users

Tapering Strategy

  • Reduce the oxcarbazepine dose by approximately 10 % per month (or more slowly) for patients discontinuing after long‑term therapy to minimize withdrawal‑related seizure risk. 1
  • Example schedule (starting at 600 mg/day ≈ 300 mg BID): decrease by ~60 mg each month (Month 1 → 540 mg/day, Month 2 → 480 mg/day, …, Month 6 → 240 mg/day) and continue a 10 % reduction of the current dose each month until cessation. 1
  • For patients who have been on oxcarbazepine ≥ 1 year, the overall taper should span several months to years, tailored to treatment duration and individual tolerance. 1

Monitoring Requirements

  • Conduct monthly follow‑up visits throughout the taper to evaluate breakthrough seizures and withdrawal symptoms. 1
  • Actively monitor for seizure recurrence: increased frequency or severity, emergence of new seizure types, or return of aura symptoms. 1
  • If clinically significant withdrawal symptoms or breakthrough seizures appear, pause the taper or further slow the reduction rate. 1

Patient‑Centered Collaboration

  • Collaborate with the patient to design the tapering plan, deciding on taper speed and any necessary pauses. 1
  • Secure patient agreement and active participation as essential determinants of successful tapering outcomes. 1
  • When clinician‑patient disagreement arises, acknowledge the discordance, express empathy, and pursue patient‑centered adjustments without abandoning care. 1

Adjustments During Taper

  • Tapers may need to be paused and later restarted; reductions often must be slowed as lower doses are approached. 1
  • For patients struggling with the taper, enhance alternative seizure‑management strategies and address behavioral distress. 1
  • Prior to reversing a taper due to breakthrough seizures, carefully discuss with the patient the benefits and risks of increasing the oxcarbazepine dose. 1

Alternative Antiepileptic Coverage & Safety

  • Optimize other antiepileptic drugs before and during the taper to maintain seizure control. 1
  • Never discontinue oxcarbazepine abruptly, as this markedly raises the risk of breakthrough seizures and status epilepticus. 1

Multidisciplinary Support

  • Involve a multidisciplinary team (nurses, pharmacists) to support the patient via telephone, telehealth, or face‑to‑face visits throughout the taper. 1

Goals of Tapering

  • Recognize that individual goals vary: some patients may achieve complete discontinuation, while others may aim for a reduced maintenance dose that balances seizure control with tolerability. 1

Common Pitfalls to Avoid

  • Avoid tapering faster than 10 % per month in long‑term users (≥ 1 year), as this increases seizure recurrence risk. 1
  • Do not initiate a taper if the patient is already experiencing increased seizure frequency or severity on the current dose. 1
