Nitrofurantoin Use in Elderly Patients with Reduced Renal Function

Renal Function Considerations

  • A GFR of 48 mL/min falls into Stage 3 Chronic Kidney Disease (moderate decrease in GFR: 30-59 mL/min/1.73m²) 1

Safety Concerns

  • Elderly patients are already at higher risk for adverse drug reactions due to age-related physiological changes and polypharmacy 2
  • Nitrofurantoin can cause serious adverse effects including pulmonary toxicity, hepatotoxicity, and peripheral neuropathy 2

Alternative Options

  • When selecting alternative antibiotics, consider local resistance patterns, patient's medication history, risk of Clostridioides difficile infection, and drug interactions with other medications 2

Special Considerations for Elderly Patients

  • Accurate assessment of renal function is critical, as serum creatinine alone may underestimate renal insufficiency in elderly patients due to decreased muscle mass 1
  • Elderly patients often have multiple comorbidities and take multiple medications, increasing the risk of drug interactions 2
