Management of Dengue in Post-PTCA Patients on DAPT

Introduction to Management Principles

  • In post-PTCA patients on DAPT who develop dengue, the European Society of Cardiology recommends continuing DAPT unless active severe bleeding occurs, as the thrombotic risk from stent thrombosis outweighs the bleeding risk in most dengue cases 1, 2

Critical Timing Considerations

  • If within 30 days of PCI, the American College of Cardiology suggests that stent thrombosis risk is highest (up to 2%), and DAPT should be maintained unless life-threatening bleeding occurs 3, 4, 5
  • If 1-6 months post-PCI, consider temporary discontinuation of one antiplatelet agent only if platelet count drops below 20,000/μL with active bleeding, as recommended by the European Heart Society 1, 2
  • If beyond 6 months post-PCI, more flexibility exists to temporarily reduce to single antiplatelet therapy during the critical dengue phase, according to the European Society of Cardiology 1, 6

Platelet Count-Based Management

  • If platelet count is 20,000-50,000/μL and less than 30 days post-PCI, continue DAPT with intensive monitoring, as suggested by the American College of Chest Physicians 3, 4
  • If platelet count is less than 20,000/μL without bleeding and less than 30 days post-PCI, continue DAPT with multidisciplinary consultation involving cardiology and hematology, as recommended by the European Heart Society 1, 2

Active Bleeding Management

  • In cases of severe bleeding, immediately discontinue both antiplatelet agents regardless of timing from PCI, as recommended by the European Society of Cardiology 1, 2

Specific Antiplatelet Agent Considerations

  • The European Heart Society suggests that if on ticagrelor or prasugrel with bleeding concerns, consider switching to clopidogrel (lower bleeding risk) 7, 8
  • Clopidogrel is preferred when balancing thrombotic and bleeding risks, according to the European Society of Cardiology 7, 9

Critical Pitfalls to Avoid

  • Never discontinue both antiplatelet agents simultaneously unless life-threatening bleeding, as this dramatically increases stent thrombosis risk, which carries 20-40% mortality, as warned by the American College of Chest Physicians 3, 4, 5

Supportive Care Measures

  • The European Society of Cardiology recommends blood pressure control to minimize bleeding risk 2
  • Proton pump inhibitor therapy (pantoprazole or dexlansoprazole) to reduce GI bleeding risk is suggested by Praxis Medical Insights 9

Resumption Strategy Post-Dengue Recovery

  • Once platelet count recovers >50,000/μL and no active bleeding, resume full DAPT immediately if within 6 months of PCI, as recommended by the European Heart Society 1, 10
