MgSO4 Loading Dose for Eclampsia Prophylaxis and Treatment
Standard Intravenous Regimen
- The American College of Obstetricians and Gynecologists recommends a magnesium sulfate loading dose of 4-6 grams IV over 20-30 minutes, followed by a maintenance infusion of 1-2 grams per hour, to achieve immediate therapeutic levels and prevent seizures in patients with preeclampsia 1, 2
- For patients with a body mass index (BMI) of 25 kg/m² or higher, the maintenance dose should be started at 2 grams per hour instead of 1 gram per hour, as this dose is more effective in achieving therapeutic levels in overweight patients, with 70-80% reaching therapeutic levels within 2-4 hours 1, 2
Alternative Pritchard Regimen (Combined IV/IM)
- The International Society for the Study of Hypertension in Pregnancy (ISSHP) endorses the Pritchard protocol, which consists of a combined loading dose of 4 grams IV plus 10 grams IM (5 grams in each buttock), followed by 5 grams IM every 4 hours in alternate buttocks for 24 hours as maintenance, as a valuable approach in resource-limited settings or when IV access is limited 3, 4
- This regimen was validated in the landmark MAGPIE trial and is particularly useful when continuous IV infusion is not feasible 3, 4
Critical Safety Considerations
- The American Heart Association recommends avoiding the combination of magnesium sulfate with calcium channel blockers, such as nifedipine, as this can cause severe hypotension and myocardial depression 1, 2, 5
- Limit total fluid intake to 60-80 mL/hour to prevent pulmonary edema in preeclamptic patients with capillary leak, and monitor for toxicity using clinical parameters such as patellar reflexes, respiratory rate, and urine output 1, 2
Common Pitfalls to Avoid
- Avoid using nonsteroidal anti-inflammatory drugs (NSAIDs) for postpartum pain in preeclamptic patients when possible, as they can worsen hypertension and increase the risk of acute kidney injury 1
- In community or lower-level settings where full protocols cannot be administered, give at least a loading dose of 5 grams IM in each buttock (10 grams total) before referral, as this is better than no treatment 3
