Pancreatic Cancer Screening and Management

Screening Recommendations

  • The European Society for Medical Oncology recommends that screening for pancreatic cancer is only indicated for individuals with specific genetic or familial risk factors, not occupational exposures 2
  • First-degree relatives from familial pancreatic cancer kindreds with at least two affected first-degree relatives should be screened 2
  • Genetic syndrome carriers, including Peutz-Jeghers syndrome, BRCA2 mutation carriers, p16 mutation carriers, Hereditary non-polyposis colorectal cancer (HNPCC) mutation carriers, and Hereditary pancreatitis syndrome, should be screened 1, 2

Diagnostic Approach

  • The American Gastroenterological Association recommends initial imaging with ultrasound of liver, bile duct, and pancreas without delay when pancreatic malignancy is suspected 4
  • Definitive staging should include Multi-detector CT (MD-CT) or MRI plus MRCP 4
  • Chest imaging with MD-CT of chest should be performed to evaluate for lung metastases 4

Treatment Recommendations

  • The National Comprehensive Cancer Network recommends radical surgery as the only curative treatment option for resectable disease (Stage I and Some Stage II) 4
  • Pancreaticoduodenectomy (with or without pylorus preservation) is recommended for pancreatic head tumors 4
  • Distal pancreatectomy with splenectomy is recommended for body and tail tumors 4
  • Surgery should be performed at high-volume specialist centers to reduce morbidity and mortality 2, 4

Palliative Management

  • The European Society for Medical Oncology recommends endoscopic stenting as the preferred method for managing biliary obstruction 5, 4
  • Metal stents are recommended for life expectancy >3 months 5, 4
  • Plastic stents should be replaced every 6 months 5
  • Opioids (morphine) are recommended as first-line treatment for severe pain, with oral route preferred 5, 4

Follow-Up After Treatment

  • The American Society of Clinical Oncology recommends CA19.9 monitoring every 3 months for 2 years for patients with elevated preoperative CA19.9 5, 4
  • Abdominal CT scan should be performed every 6 months 5, 4
  • Toxicity assessment should be performed at each cycle for patients on chemotherapy 6
  • Response evaluation with imaging should be performed every 8 weeks 6
