Management of Chronic Hypoglycemia in Non-Diabetic Adults

Diagnostic Confirmation

  • The American Diabetes Association recommends confirming true hypoglycemia by measuring low plasma glucose ≤54 mg/dL during symptoms, as stated in the Diabetes Care journal 1

Acute Episode Management

  • For conscious patients, the American Diabetes Association suggests administering 15-20 grams of fast-acting carbohydrates immediately when glucose drops to 70 mg/dL or below, as recommended in the Diabetes Care journal 2, 3
  • For severe episodes with altered mental status, the American Diabetes Association recommends administering glucagon 1 mg intramuscularly, as stated in the Praxis Medical Insights journal 4

Systematic Diagnostic Approach

  • The American Diabetes Association advises obtaining critical blood samples during hypoglycemia, including laboratory glucose, insulin level, C-peptide, proinsulin, and beta-hydroxybutyrate, as recommended in the Diabetes Care journal 1, 3, 4

Specific Cause-Directed Management

  • The American Diabetes Association suggests that medication-induced hypoglycemia requires immediate discontinuation of the causative agent, as stated in the Diabetes Care journal 5

Ongoing Prevention Strategy

  • The American Diabetes Association recommends consuming frequent small meals with complex carbohydrates and adequate protein, and eliminating refined sugars, as stated in the Diabetes Care journal 1
  • The American Diabetes Association suggests using continuous glucose monitoring with hypoglycemia alerts, as recommended in the Metabolism: Clinical and Experimental and Diabetologia journals 6, 7

Emergency Preparedness

  • The American Diabetes Association advises prescribing glucagon for all patients with a history of severe hypoglycemia, as stated in the Diabetes Care and Praxis Medical Insights journals 3, 4

Follow-Up Surveillance

  • The American Diabetes Association recommends assessing hypoglycemia history at every visit, including frequency, severity, precipitants, symptoms, and treatment response, as stated in the Diabetes Care journal 2, 5
