COVID-19 Treatment Approach

Introduction to Treatment Strategy

  • The primary treatment approach for COVID-19 is supportive care, with targeted therapies based on disease severity, including remdesivir for hospitalized patients and those at high risk for progression, dexamethasone for patients requiring oxygen, and anti-SARS-CoV-2 monoclonal antibodies for high-risk patients with mild to moderate disease 1, 2

Treatment Based on Disease Severity

  • For patients with mild symptoms not requiring hospitalization but at high risk for progression, anti-SARS-CoV-2 monoclonal antibodies are recommended, especially for unvaccinated individuals or those with impaired immune response 1
  • High-titer convalescent plasma within 72 hours from symptom onset is recommended if monoclonal antibodies are unavailable 1
  • Nirmatrelvir/ritonavir (oral antiviral) can be considered for patients with mild COVID-19 1
  • Molnupiravir may be considered when other options are unavailable for patients with mild COVID-19 1
  • For moderate COVID-19, dexamethasone 6 mg daily for up to 10 days is recommended for patients requiring oxygen 2, 3
  • For severe/critical COVID-19, dexamethasone 6 mg daily for up to 10 days is recommended 1, 3
  • Remdesivir may have limited benefit in critically ill patients on mechanical ventilation 1

Supportive Care Measures

  • Oxygen supplementation to maintain SpO2 >90-96% is recommended 2
  • Careful fluid management is recommended 2
  • Thromboprophylaxis with anticoagulation is recommended by the American College of Cardiology 6
  • Monitor and treat co-infections or superinfections as needed 2

Special Considerations

  • Pre-exposure prophylaxis with long-acting monoclonal antibodies is recommended for unimmunized immunocompromised patients at risk for severe COVID-19 1
  • Post-exposure prophylaxis with monoclonal antibodies is recommended for high-risk immunocompromised patients (unvaccinated, vaccine non-responders) 1
  • Consider longer treatment duration with antivirals for immunocompromised patients 1
  • Treatment decisions should consider the patient's age, comorbidities, and disease progression 7
  • For patients with COVID-19 and concurrent hematological malignancies, treatment should be tailored to their specific risk factors 1

Treatment Duration

  • Dexamethasone is typically administered for up to 10 days or until hospital discharge 3
