HCG Dosing and Benefits in Assisted Reproductive Technology and Male Hypogonadotropic Hypogonadism

  • The American Society for Reproductive Medicine recommends administering 5,000 IU of hCG intramuscularly when at least three follicles reach >17 mm diameter with appropriately rising estradiol levels for ovulation induction in assisted reproductive technology 1
  • The standard dose for ovulation induction is 5,000 IU hCG administered intramuscularly or subcutaneously when follicular maturation criteria are met, as recommended by the European Society of Human Reproduction and Embryology 1, 2, 3, 4, 5
  • For male hypogonadotropic hypogonadism, initial therapy with hCG 500-2,500 IU administered 2-3 times weekly is recommended by the Endocrine Society 6

Clinical Benefits

  • The use of hCG in assisted reproductive technology increases pregnancy rates from 8.4% with monofollicular growth to 15% with multifollicular development, as reported by the American College of Obstetricians and Gynecologists 2, 3, 4, 5
  • hCG effectively normalizes testosterone levels in men with deficient LH and FSH secretion, as stated by the American Urological Association 7, 6

Critical Safety Considerations

  • The American College of Obstetricians and Gynecologists recommends withholding hCG administration when >2 dominant follicles >15 mm OR >5 follicles >10 mm are present to prevent high-order multiple gestations 2, 3, 4, 5, 8
  • The Endocrine Society warns that hCG use in males currently on or planning exogenous testosterone therapy may suppress gonadotropin secretion and negate hCG benefits 6

Monitoring Requirements

  • The National Comprehensive Cancer Network recommends measuring serum beta-hCG at least every 1-2 weeks during chemotherapy for gestational trophoblastic neoplasia 9
  • The American Urological Association suggests monitoring serum testosterone response before adding FSH in males with hypogonadotropic hypogonadism 7, 6

hCG Administration for Fertility Treatment

Dosing and Timing

  • The Human Reproduction Update recommends a timing window of 24-40 hours post-hCG for single insemination without compromising pregnancy rates in IUI with ovarian stimulation 10
  • The Endocrine Society recommends an initial therapy of 500-2,500 IU administered 2-3 times weekly for male hypogonadotropic hypogonadism, with monitoring of serum testosterone response before adding FSH analogues 11

Clinical Considerations

  • The American College of Obstetricians and Gynecologists and other fertility guidelines are not mentioned, however, the provided information from the Human Reproduction Update and the Endocrine Society can be used to inform clinical decisions 10, 11
