Drug Interaction Between Prednisone and Xarelto (Rivaroxaban)

Primary Interaction Mechanism

  • The American Academy of Family Physicians and the European Society of Cardiology recommend avoiding the combination of prednisone with Xarelto (rivaroxaban) whenever possible, as corticosteroids significantly increase bleeding risk when used with anticoagulants, particularly in patients with renal impairment, liver dysfunction, or bleeding history 1, 2
  • The combination of corticosteroids with anticoagulants creates a 3-6 fold increase in gastrointestinal bleeding risk compared to anticoagulants alone, due to impaired gastric mucosal protection, peptic ulceration, and antiplatelet effects of corticosteroids 1
  • Both drugs independently increase bleeding risk, creating additive or synergistic effects 2

High-Risk Patient Populations Requiring Extra Caution

  • The American College of Cardiology recommends that rivaroxaban is contraindicated in patients with Child-Pugh B or C hepatic impairment, especially when combined with corticosteroids, due to increased bleeding risk 5
  • Patients with a history of upper GI bleeding have a 5% recurrence risk within 6 months when using anticoagulants, and combining with corticosteroids further amplifies this risk 1, 2
  • Elderly patients (≥75 years) on rivaroxaban have particularly increased gastrointestinal bleeding rates compared to warfarin 2, 4

Management Strategies When Combination Cannot Be Avoided

  • The European Heart Society recommends adding a proton pump inhibitor (PPI) to all patients receiving both prednisone and rivaroxaban to reduce endoscopically-diagnosed ulcers and bleeding risk 1, 2
  • Implementing frequent clinical assessment with specific bleeding surveillance, including education on bleeding signs, monitoring hemoglobin/hematocrit, and checking renal function, is crucial when combining prednisone and rivaroxaban 2, 5, 6

Alternative Anticoagulation Options

  • The European Society of Cardiology suggests considering alternatives to rivaroxaban with better monitoring capability, such as unfractionated heparin with aPTT monitoring, low molecular weight heparin with anti-factor Xa monitoring, or warfarin with INR monitoring, in patients requiring chronic corticosteroids 5, 4

Critical Drug-Drug Interaction Considerations

  • The American Heart Association recommends avoiding additional medications that compound bleeding risk, such as NSAIDs, antiplatelet agents, and combined P-gp and strong CYP3A4 inhibitors, when using rivaroxaban with prednisone 1, 2, 6
