Fluconazole Treatment Guidelines

Introduction to Fluconazole Presentations

  • The American College of Physicians recommends the use of fluconazole in various presentations, including oral tablets and intravenous formulations, with dosages varying based on the clinical indication, and the most commonly used tablets are 100-400 mg for Candida infections 1

Dosage Recommendations

  • For oropharyngeal candidiasis, the initial dose is 200 mg on the first day, followed by a maintenance dose of 100 mg daily for 7-14 days, with oral tablets or suspension being the recommended presentation 1, 3
  • For esophageal candidiasis, the initial dose is 200 mg on the first day, followed by a maintenance dose of 100-400 mg daily for 14-21 days, with oral tablets or intravenous formulation being the recommended presentation if the patient cannot tolerate oral therapy 1, 2, 3
  • For candidemia and systemic candidiasis, the loading dose is 800 mg (12 mg/kg), followed by a maintenance dose of 400 mg (6 mg/kg) daily, with intravenous formulation being the recommended initial presentation 3, 4

Special Considerations

  • In cases of C. glabrata or C. krusei infections, alternative treatments should be considered due to intrinsic or acquired resistance 4, 5
  • The choice of presentation should consider the severity of the infection, the patient's ability to tolerate oral medication, and the susceptibility pattern of the isolated pathogen 1, 2
