Olmesartan Dosing for Hypertension in Adults

Initial Dosing and Titration

  • The American College of Physicians recommends monitoring blood pressure regularly during titration, with follow-up at monthly intervals until control is achieved 1, 2
  • The European Society of Cardiology recommends targeting blood pressure to <140/90 mmHg for all patients with hypertension without comorbidities, and <130/80 mmHg for high-risk patients with existing cardiovascular disease, diabetes, or chronic kidney disease 3, 4, 1

Blood Pressure Management

  • The American Heart Association recommends that if blood pressure remains uncontrolled on olmesartan 40 mg daily, a second antihypertensive agent from a different class should be added 1, 4
  • The European Society of Hypertension recommends that for stage 2 hypertension (BP ≥160/100 mmHg), combination therapy with two agents from different classes should be initiated at the outset 1, 2

Resistant Hypertension Management

  • The American College of Cardiology recommends adding spironolactone (mineralocorticoid receptor antagonist) as the fourth agent if blood pressure remains ≥130/80 mmHg despite olmesartan at maximum dose plus two other agents (including a diuretic) 3, 2

Special Considerations

  • The American Geriatrics Society recommends initiating therapy cautiously and monitoring closely for orthostatic hypotension, especially when using two drugs initially in elderly patients (≥65 years) 2
  • The National Kidney Foundation recommends assessing electrolytes and renal function 2-4 weeks after initiating therapy, particularly when combining with diuretics 1
