Differentiating Cecal Diverticulitis from Ruptured Pelvic Appendicitis

Clinical Presentation and Diagnostic Features

  • Patients with appendicitis typically present with more acute onset of symptoms, often with migration of pain to the right lower quadrant 4
  • Laboratory findings may show elevated inflammatory markers in both conditions, with CRP >50 mg/L being a significant marker for diverticulitis 1

CT Imaging Features

  • The American College of Radiology recommends CT imaging with intravenous contrast for differentiating cecal diverticulitis from ruptured pelvic appendicitis, as it provides the highest diagnostic accuracy for both conditions 1, 2, 3
  • Presence of inflamed diverticulum (outpouching) from the cecal wall is a characteristic feature of cecal diverticulitis 1, 2
  • Thickening of the colonic wall with increased density of pericolic fat is also a feature of cecal diverticulitis 2
  • Extraluminal gas, focal wall defect, and circumferential periappendiceal inflammatory changes are characteristic of ruptured pelvic appendicitis 4
  • Appendiceal diameter >11 mm has high sensitivity for perforation 4
  • Pelvic abscess formation and free fluid in the pelvis are also features of ruptured pelvic appendicitis 2, 4

Diagnostic Algorithm and Management

  • Elevated WBC and CRP are present in both conditions, with CRP >170 mg/L suggesting more severe disease in diverticulitis 1, 6
  • The American College of Radiology recommends performing abdominal CT with IV contrast for highest diagnostic accuracy 3, 7, 8
  • Uncomplicated cecal diverticulitis may be managed conservatively with antibiotics 1, 2
  • Ruptured pelvic appendicitis requires surgical intervention 4

Important Considerations

  • Perforated colon cancer can mimic both cecal diverticulitis and appendicitis; presence of pericolonic lymphadenopathy (>1 cm in short axis) suggests malignancy 5
