Mounjaro (tirzepatide) Dosing and Titration Guidelines

Titration Schedule

  • The FDA‑approved titration schedule escalates the dose by 2.5 mg every 4 weeks, beginning with 2.5 mg weekly and progressing through 5 mg, 7.5 mg, 10 mg, 12.5 mg, to a maximum of 15 mg weekly. 2

Criteria for Dose Advancement

  • Advance to the next dose only after a minimum of 4 weeks on the current dose and when the individualized HbA1c target (generally < 7.0 % for most adults with type 2 diabetes) has not been reached. 3
  • The current dose must be tolerable; gastrointestinal symptoms such as nausea, vomiting, or diarrhea should have resolved or be only mild before escalation. 1
  • For patients using tirzepatide for obesity, higher doses yield greater weight loss: 10 mg produces ~19.5 % mean loss and 15 mg ~20.9 % loss at 72 weeks. 1

Timing Rules & Dose Management

  • Never accelerate titration faster than every 4 weeks; shortening the interval markedly raises gastrointestinal adverse events and the risk of treatment discontinuation. 1
  • If treatment is interrupted for any duration, restart at 2.5 mg and repeat the full titration schedule to avoid severe gastrointestinal intolerance. 1

Maintenance Dosing

  • FDA‑approved maintenance doses are 5 mg, 10 mg, and 15 mg once weekly, with 15 mg being the maximum allowable dose. 1

Common Pitfalls to Avoid

  • Skipping the initial 2.5 mg starting dose increases the likelihood of clinically significant gastrointestinal side effects. 1
  • When initiating tirzepatide, reduce concurrent insulin doses by 20–30 % and consider discontinuing sulfonylureas to prevent hypoglycemia. 1

Special Populations

  • Advanced chronic kidney disease (eGFR < 30 mL/min/1.73 m²): No dose adjustment is required; tirzepatide is preferred over insulin because it carries a lower hypoglycemia risk. 4
  • Type 2 diabetes with obesity: Weight‑loss efficacy is modestly lower than in obesity alone, with mean reductions of ~12.8 % (10 mg) and ~14.7 % (15 mg) at 72 weeks (SURMOUNT‑2 trial). 1
  • Heart failure with preserved ejection fraction (HFpEF) and obesity: The SUMMIT trial employed the same escalation schedule (2.5 → 5 → 7.5 → 10 → 12.5 → 15 mg, each for 4 weeks). 2, 5
