Diagnostic and Treatment Considerations for Cryptogenic Organizing Pneumonia
Diagnostic Criteria
- The American Thoracic Society and European Respiratory Society guidelines characterize the organizing pneumonia pattern by a patchy process involving alveolar ducts and alveoli with or without bronchiolar intraluminal polyps 1
- The 2020 ATS/JRS/ALAT guidelines require three features for a definitive histopathological diagnosis of nonfibrotic COP: cellular interstitial pneumonia, cellular bronchiolitis, and poorly formed nonnecrotizing granulomas 2
Treatment and Prognosis
- The presence of Masson bodies confirms organizing pneumonia and predicts an excellent response to oral corticosteroids in the majority of patients, with typical initial dosing of prednisone ~50 mg/day 1, 3
- Complete recovery occurs in the majority of patients, though relapse is common, reported in up to two-thirds of cases 1
- The 5-year survival for cryptogenic OP is 73%, compared to 44% for secondary OP 4
Diagnostic Approach
- Correlate biopsy findings with HRCT findings, including patchy migratory consolidation and reversed halo sign 1, 3
- Confirm subacute presentation with median duration <3 months and symptoms such as cough, dyspnea, and fever 1, 3
- Exclude secondary causes, including collagen vascular diseases, infections, malignancies, drug reactions, and hypersensitivity pneumonitis, before diagnosing cryptogenic OP 3, 5
