Paracetamol Use in Chronic Kidney Disease

General Guidelines

  • The Kidney International society recommends using standard therapeutic doses of paracetamol (up to 4 grams daily in divided doses) for patients with CKD stages 1-3a (eGFR ≥45 mL/min/1.73 m²), with no routine dose adjustment required 1, 2
  • The Kidney International society suggests monitoring eGFR and electrolytes periodically as part of routine CKD care for patients with CKD stages 1-3a (eGFR ≥45 mL/min/1.73 m²) 2, 3, 4

Dosing Recommendations

  • For CKD stages 3b-4 (eGFR 15-44 mL/min/1.73 m²), standard doses of paracetamol are generally safe, but increased monitoring frequency is recommended 1
  • For CKD stage 5 (eGFR <15 mL/min/1.73 m²), paracetamol should be used with caution, considering a 50% dose reduction or extended dosing intervals 1

Monitoring Parameters

  • The Kidney International society recommends baseline and periodic eGFR monitoring using validated eGFR equations for patients with CKD 2, 6, 7
  • Electrolyte monitoring is also recommended, particularly in patients on multiple medications 2, 3, 4

Drug Stewardship

  • The Kidney International society suggests reviewing and limiting over-the-counter medicines that patients may be taking concurrently, as paracetamol is present in many combination products 2, 3, 4, 6
  • A thorough medication review should be performed periodically and at transitions of care 2, 5, 7
